Tinnitus & Audiological Management

TINNITUS: Sensation of sound without external stimulation. It is the perception of sound that results from activity within the cochlea and not related to external stimulation of any kind.

Tinnitus & Audiological Management combines specialized expertise to address tinnitus and hearing concerns. Our integrated approach ensures comprehensive evaluation and tailored treatments for optimal outcomes, fostering better hearing and improved quality of life.

When dealing with tinnitus, it’s important to seek professional help from the best tinnitus clinic and specialists. A tinnitus specialist, often a doctor with expertise in audiology or otolaryngology, can provide accurate diagnosis and effective management strategies tailored to each individual. At the best tinnitus clinic, these specialists employ advanced diagnostic tools and personalized treatment approaches to alleviate symptoms and improve the overall quality of life for those affected by tinnitus.

Tinnitus - Evaluation & Management

  • Audiological Evaluation
  • Psycho Acoustical Evaluation (pitch & loudness matching)
  • Medical Evaluation

  • Tinnitus Retraining Therapy (TRT)
  • Masking
  • Medication
  • Psychological treatments
  • Electric stimulation
  • Biofeedback training
  • Surgical technique
audiology treatment

  • Vasodilators
  • Local anaesthetics
  • Sedatives & tranquilizers
  • Antidepressants
  • Antiallergic

SURGICAL TECHNIQUE: Micro Vascular Decompression (MVD), surgery of the VIII cranial nerve for the relief of vertigo & disequilibrium also reduces tinnitus.

TRT techniques are based on neurophysiological model.

Neurophysiological Model: A novel sound induces a new pattern of activity with the auditory pathways & subcortical centres allowing it to reach the cortex.

This process is habituation of reaction with repetitive appearance of a sound. The sub cortical pathway will block it and the individual will be unaware that the sound is present. This is the process of habituation of perception.

Tinnitus Habituation Therapy: Based on brain plasticity and its ability to habituate to a variety of stimuli. Change in the synaptic connections between neurons throughout the life facilitates learning. The auditory system and its connection with limbic system can be retrained. By proper retraining procedures, tinnitus can be blocked, thereby preventing the activation of systems.

Therefore, habituation of tinnitus is achieved by filtering out and blocking tinnitus related neuronal activity from reaching the brain.

Achieving habituation consists of:

  • Counselling
  • TRT

Aims of counselling:

  • Tinnitus does not reflect medical problems.
  • Tinnitus as a problem arises due to enhanced activation of emotional and ANS centres in brain.
  • Utilize normal functions of the brain to attenuate these abnormal activation, thus inhibition.
  • Explaining why tinnitus & hyperacusis create problems.
  • Discussion about proposed treatment.
  • Avoidance of silence.
  • Enriching the background sound.
  • Masking prevents habituation.
  • Stress can worsen tinnitus.

  • Introductory contact
  • Initial visit
  • Tracking between tinnitus
  • Sound tolerance
  • Hearing loss
  • Audiological evaluation
  • Medical evaluation
  • Assessing the category for treatment
  • Counselling, introduction to sound therapy
  • Fitting of instruments
  • Follow up visits.
  • Closing treatment

Classifying Tinnitus

Category 0:

  • Low level of tinnitus severity
  • One session of simplified general counselling

Category 2:

  • Sever tinnitus co existing with subjective hearing loss.
  • Appropriate counselling & sound therapy
  • Fitting of appropriate hearing aids

Category 4:

  • Significant hyperacusis with tinnitus as secondary complaint or absent
  • Result of noise exposure
  • Treatment approach to remove hyperacusis.
  • Difficult to treat with sound therapy in normal way and requires a protocol based on very slow desensitization.

Category 1:

  • High severity but do not have hyperacusis, subjective hearing loss.
  • Detailed counselling focused on tinnitus.

Category 3:

  • Tinnitus of high severity with hyperacusis, subjective hearing loss
  • This is not enhanced by prolonged period of time as a result of sound exposure
  • Sound generators are necessary to help to desensitize hyperacusis
  • Appropriate counselling & sound therapy
  • Appropriate hearing aid

Frequently Asked Questiones

“You can put anything into your ears to clean them as long it is bigger than your elbow!” Yes, it’s true – your ears do not require cleaning, wax dose not need to be removed – it is a protective secretion of the ear canal.
This is related to sudden change in temperature, which may lead to sneezing or watery discharge from the nose. As a precaution avoid sudden changes in temperature e.g. switch off the car AC a minute before you are to get off, similarly with the room AC.
For constant irritation/ general dryness in the throat kindly gargle with warm saline once after brushing your teeth in the morning and once just before going to sleep at night.
