TINNITUS RETRAINING THERAPY (TRT)

TRT techniques are based on neurophysiological model.

Neurophysiological Model: A novel sound induces a new pattern of activity with the auditory pathways & subcortical centres allowing it to reach the cortex.

This process is habituation of reaction with repetitive appearance of a sound. The sub cortical pathway will block it and the individual will be unaware that the sound is present. This is the process of habituation of perception.

Tinnitus Habituation Therapy: Based on brain plasticity and its ability to habituate to a variety of stimuli. Change in the synaptic connections between neurons throughout the life facilitates learning. The auditory system and its connection with limbic system can be retrained. By proper retraining procedures, tinnitus can be blocked, thereby preventing the activation of systems.

Therefore, habituation of tinnitus is achieved by filtering out and blocking tinnitus related neuronal activity from reaching the brain.

Achieving habituation consists of:

Aims of counselling:

  • Tinnitus does not reflect medical problems.
  • Tinnitus as a problem arises due to enhanced activation of emotional and ANS centres in brain.
  • Utilize normal functions of the brain to attenuate these abnormal activation, thus inhibition.
  • Explaining why tinnitus & hyperacusis create problems.
  • Discussion about proposed treatment.
  • Avoidance of silence.

  • Masking prevents habituation.
  • Stress can worsen tinnitus.

Classifying Tinnitus

Category 0:

  • Low level of tinnitus severity
  • One session of simplified general counselling

Category 2:

  • Sever tinnitus co existing with subjective hearing loss.
  • Appropriate counselling & sound therapy
  • Fitting of appropriate hearing aids

Category 4:

  • Significant hyperacusis with tinnitus as secondary complaint or absent
  • Result of noise exposure
  • Treatment approach to remove hyperacusis.
  • Difficult to treat with sound therapy in normal way and requires a protocol based on very slow desensitization.

Category 1:

  • High severity but do not have hyperacusis, subjective hearing loss.
  • Detailed counselling focused on tinnitus.

Category 3:

  • Tinnitus of high severity with hyperacusis, subjective hearing loss
  • This is not enhanced by prolonged period of time as a result of sound exposure
  • Sound generators are necessary to help to desensitize hyperacusis
  • Appropriate counselling & sound therapy
  • Appropriate hearing aid

This is related to sudden change in temperature, which may lead to sneezing or watery discharge from the nose. As a precaution avoid sudden changes in temperature e.g. switch off the car AC a minute before you are to get off, similarly with the room AC.
For constant irritation/ general dryness in the throat kindly gargle with warm saline once after brushing your teeth in the morning and once just before going to sleep at night.
